Appointment Instructions

To schedule an appointment at SHAC, call (505) 277-3136.


If you are being seen in the clinic:

Please arrive 15 minutes prior to your appointment to allow for building access and the screening process. Only the patient being seen by a SHAC provider will be allowed in the building. 

Sign in at the iPads at the Reception Area on the first floor. Or, you can sign in with our touchless, contactless "Mobile Check-In App." To download the app, visit Google Play or Apple App Store.

Be sure to wear a mask or face covering. If you don't have one, we can provide one for you at a minimal cost. Also, please bring a valid picture ID and Insurance card. 


If you received a Zoom invitation for your appointment:

Your provider will call or connect with you as close to the appointment time as possible. Please be available and in a private place to get the most out of your appointment. Note: Charges may apply for Telehealth Services.


Cancellations:

If you need to cancel an appointment, please call Student Health and Counseling by 3:00 PM the day prior to your appointment. A $20 No-Show Fee for missed in-person and telehealth appointments will be assessed to your student account if you fail to cancel your appointment by 3:00 PM the previous business day before your appointment.

If we have made an error in scheduling you or you believe you deserve special consideration for a “no-show” or “late cancellation” fee, please complete the No-Show / Late Cancelation Petition.


New Patient or an Incoming Health Sciences Student:

Please complete your new patient paperwork online prior to your visit. Go to the the SHAC Health Portal and sign in using your UNM NetID and password. Next to your upcoming appointment, there will be a link to complete the required forms.
