What Are Advance Directives and Who Needs Them?
SHAC has made available two optional Advance Directives; both are legal documents governed by New Mexico state statute. Patients who plan to communicate their wishes, should they become medically or psychiatrically incapacitated, have Advance Directives as a supportive measure.
To submit a Medical and/or Mental Health Advance Directive(s), download the form below, complete, and upload to the SHAC Health Portal. The final step of this process is to have a conversation with your SHAC health and/or mental health providers. Call SHAC at (505) 277-3136 to schedule an appointment.
